Oxford Valley Dental Excellence
Smiledog Millonaire –Smile Makeover Search Contest
To enter this sweepstakes, you must agree to and abode by these Official rules.
NO PURCHASE NECESSARY TO ENTER OR WIN. Entries that are incomplete, illegible, fraudulent or corrupted are void and will not be accepted.

2. Sweepstakes Schedule: The Sweepstakes begins at 07:00:01 pm Eastern Time ("ET") on 4/30/2009 and ends at 11:59:59 p.m. ET on 5/29/09
3. ELIGIBILITY: Eligibility: The SmileDog Millionaire Smile Makeover Search is open only to legal residents of the 50 United States and Washington D.C., who are at least 18 years of age and who have an active e-mail account and Internet access prior to the beginning of the Promotion Period. Employees of Oxford Valley Dental Excellence, any organizations responsible for sponsoring, fulfilling or administering the Sweepstakes or supplying the prizes, persons involved in the creation, development or production of the Smiledog Millionaire Smile Makeover Search, and immediate family and household members of such individuals, are not eligible to enter or win. "Immediate family members" shall mean parents, step-parents, children, step-children, siblings, step-siblings, or spouses, regardless of where they live. "Household members" shall mean people who share the same residence at least three months a year, whether related or not. Potential winners may be required to provide proof of eligibility upon request prior to prize award. Void in Puerto Rico and where prohibited or restricted by law.
4. Sweepstakes Entry: To enter, go to http://www.SmileDogMillionaire.com and click sign me up.
For a chance to win a the grand prize of up to $30,000 in restorative and cosmetic dentistry by May 31st, 2009, you must submit your entry online by 11:49 p.m. ET on Saturday, May 29th, 2009. All contestants will be notified by Oxford valley Dental Excellence and are required to schedule a hassle free contest exam where you will be become officially enrolled in the Smiledog Millionaire Contest. During your hassle free consultation you will receive free dental exam and consultation, a set of intra-oral photos, Panorex x-rays, and a before and after simulation valued at over $500. You will also be asked to fill out an essay on why you deserve a smile makeover.
Oxford Valley Dental Excellence (“Sponsor”), father and son dentists specializing in restorative and cosmetic dentistry, including a celebrity panel will judge and announce the finalists on Tuesday June 9th, 2009. Finalists will be notified via e-mail and a reception will be held at Oxford Valley Dental Excellence on or about Wednesday June 10th, 2009.
All entries become the property of Sponsors and will not be acknowledged.
You agree to consent to receive promotional e-mails and reminder e-mails from Oxford Valley Dental Excellence. The use of such e-mails shall be subject to the privacy policies of Oxford Valley Dental Excellence you may opt out of receiving these e-mails at any time. See Section 11 below for more information. Limit one (1) entry per person or e-mail address, per day. Multiple entries in excess of this limitation or mass entry attempts will result in disqualification of all entries by such person. Entries must be manually key-stroked; use of automated entry devices or programs is prohibited.
5. Random Drawing/Selection of Winner: On or about Wednesday June 9th, 2009, the (3) Three finalist will be voted on and selected by the Oxford Valley Dental Excellence panel of judges.
Potential winners will be notified by e-mail. Failure to respond to the initial verification e-mail within three (3) business days after notification or return of e-mail notification as undeliverable after three (3) attempts will result in disqualification. Potential winner will then be required to sign and return within five (5) business days an Affidavit of Eligibility, Release and Indemnification, and a Publicity Release (collectively, the "Prize Claim Documents") where allowed by law. If the Prize Claim Documents are not returned to Sponsors within the specified time period, the potential winner will be disqualified. If winner is disqualified, prize may be awarded to an alternate winner randomly selected from all remaining eligible entries. In the event of any dispute concerning the identity of any entrant, the entry will be deemed submitted by the natural person who is the authorized account holder of the transmitting e-mail account at the time of entry. "Authorized account holder" is defined as the natural person who is assigned to the e-mail address by an Internet access provider, online service provider, or other organization (e.g., business, educational institution, etc.) that is responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address.
Sponsors and their agencies are not responsible for failed, partial or garbled computer transmissions, or for technical failures of any kind, including but not limited to electronic malfunction or damage of any network, hardware or software. If for any reason the Sweepstakes is not capable of running as planned, including infection by computer virus, bugs, tampering, unauthorized intervention, fraud or technical failures beyond Sponsors' control that corrupt or affect the administration, security, fairness, integrity or proper conduct of the Sweepstakes, Sponsors reserves the right, in their sole discretion, to disqualify any individual who tampers with the entry process, and to terminate, modify or suspend the Sweepstakes. In such event, Sponsors reserve the right to conduct the Sweepstakes and award the prize by means of a random drawing comprised of all eligible entries received prior to and/or after (as appropriate) the action taken by Sponsors or otherwise award the prize in a manner which is fair, appropriate and consistent with these Official Rules, as determined by Sponsors in their sole discretion. Notice of termination/modification/suspension of Sweepstakes will be posted on the website. No responsibility is assumed for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or loss, destruction or unauthorized access to, or alteration of, entries; or any problems or technical malfunctions of any telephone network or lines, computer online systems, servers, or providers, computer equipment, software, failure of any e-mail or electronic entry to be received on account of technical problems or traffic congestion on the Internet or at any website, or any combination thereof, including any injury or damage to entrant's or any other person's computer related to or resulting from participation in or downloading any materials from this Sweepstakes.
Entrants further agree not to knowingly damage or cause interruption of the Sweepstakes and/or prevent others from participating in the Sweepstakes. CAUTION: ANY ATTEMPT TO DAMAGE ANY ONLINE SERVICE OR WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES VIOLATES CRIMINAL OR CIVIL LAWS. IF SUCH AN ATTEMPT IS MADE, SPONSORS MAY DISQUALIFY ANY ENTRANT MAKING SUCH ATTEMPT AND SEEK DAMAGES TO THE FULLEST EXTENT OF THE LAW.
6. Prize, Approximate Retail Value ("ARV") and Odds of Winning: One (1) Grand Prizewinner will receive a credit up to but not limited to $30,000 in smile makeover procedures.
One (1) second place winner will receive a credit up to but not limited to $5,000 in smile makeover procedures.
One (1) third place winner will receive a credit up to but not limited to $2,500 in smile makeover procedures.
These procedures will include crowns, bridges, veneers, implants, and any additional services offered by Oxford Valley Dental Excellence.
Total value of all prizes awarded: $37,500.00.
No substitution or transfer of prize except with Sponsors' permission or where required by law. If for any reason announced prize becomes unavailable, Sponsors in their sole discretion may award a substitute prize of equal or greater value. Prize will only be delivered to address within the United States. Federal, state and local taxes, if any, are the sole responsibility of the winner, who will receive a 1099 tax form reflecting actual value of any prize valued at $600 or more. Sweepstakes available to over 100 million Internet users. Odds of winning Grand Prize depend on total number of eligible entries received during the Promotion Period.
7. Conditions and Limitations of Liability: By participating in this Contests, entrants agree to be bound by these Official Rules and the decisions of Sponsors, which shall be final in all respects. By participating in this Sweepstakes, entrants agree to release and hold harmless Sponsors, Administrator, and each of their respective parent, subsidiary, affiliated and successor companies, advertising and promotion agencies and prize suppliers, and each of their respective officers, directors, agents, representatives and employees, as well as each of their respective successors, representatives and assigns (collectively, the "Released Parties") from and against any and all claims or actions of any kind, including but not limited to any claim related to entrants submission of an entry, entrants participation in the Sweepstakes, or for property damage, product liability, wrongful death or personal injury arising out of or relating to possession, acceptance, receipt or use/misuse of the prize, whether under contract, tort or any other legal theory, and agree to indemnify the Released Parties against any liability or expense (including reasonable attorneys' fees) arising in connection with the foregoing.
By entering, entrants also authorize the Released Parties to use their name, voice, likeness, photograph, prize information, biographical data and city and state of residence in programming or promotional material or on a winner's list (if applicable) without further compensation unless prohibited by law. Sponsors are not obligated to use any of the above mentioned information or materials, but may do so and may edit such information or materials, at Sponsors; sole discretion, without further obligation or compensation. All entries become the property of Sponsors and will not be acknowledged.
8. Winners List: For the name of the winner, mail a self-addressed, stamped envelope to "Smiledog Millionaire Smile Makeover Search", c/o Chad Cushner, 240 Middletown Blvd, Suite 100, Langhorne, PA 19047 by June 30, 2009.
9. Governing Law: Sweepstakes subject to the laws of the United States only. All federal, state and local laws and regulations apply. The Sweepstakes and Official Rules are interpreted by the laws of California, without regard to its conflict of law provisions.
